Intrapleural Administration of Hydrocortisone for T.B. Pleural Effusion


Abstract
In order to evaluate the effecacy of intrapleural administration of hydrocortisone for the treatment of T. B. pleural effusion, 42 patients with T. B. pleural effusion were studied.
1. The study is comprised of two groups: Of 42 patients, 22 cases were given hydrocortisone intrapleurally with INH-PAS, 20 cases, the remaining, being given INH-PAS alone.
2. In the former group, the disease course apparently shortened: Relatively quick amelioration of general condition, subsidence of fever, weight gain, early improvement of ESR, quickening of pleural effusion absorption, and less pleural thickening were observed, in comparison to the latter group.
3. The study implies that intrapleural instillation of hydrocortisone is of significant value in the treatment of TB. pleural effusion and combined treatment of hydrocortisone with INH-PAS is superior to single treatment. with INH-PAS alone.
4. A large dose of hydrocortisone is required for good result in this study as a topical application, compared to other systemic use. For an intrapleural adrninistration of 250mg seems proper and an interval of 2 weeks interrupted period of the agent is recommended if repeated administration of the agent is desired.
